HIV & AIDS Transitional Case Management

Overview
The LINKS Program assists people living with HIV/AIDS in Santa Clara County Adult Custody to make a smooth transition from jail to the community.
- A transitional case manager will meet with each client to determine their eligibility, assess their needs and assist with release planning.
- Upon release, the transitional case manager maintains contact with the client to monitor progress and provide assistance with referrals.
- Referrals can include medical care, medication coverage programs, Food Basket, transportation services, counseling and/or substance abuse treatment, housing and long-term case management as needed.
Program Eligibility
- Must be currently or recently incarcerated at Elmwood or Main Jail
- Must have verifiable, documented HIV diagnosis
- Must be capable/competent to participate in own plan of care and follow-up
- Must be a Santa Clara County resident
- Clients will need to have a picture ID upon release in order to obtain certain services in the community
LINKS will assist with housing information or referrals, but clients are not given or guaranteed
placement upon release. If clients are already
established with The Health Trust, they need only
to call (408) 961-9850 to restart services.
