Health Insurance Enrollment
To help ensure all children in Santa Clara County have access to quality and affordable health care coverage, The Health Trust conducts insurance outreach and enrollment. Most of our clients are children in Santa Clara County who are uninsured and in families with income levels at or below 300% FPL ($60,000 for family of four). Since 2001, we have assisted over 25,000 children enroll or continue their enrollment in a health insurance program. We are currently the third largest Enrollment Entity (State designation for agencies employing Certified Application Assistors) in Santa Clara County. We currently generate approximately 1,300 new applications and 750 renewals annually. Through an effective outreach strategy and culturally sensitive work-plan, our certified enrollers work to reduce health access barriers by providing the uninsured with application assistance and health education regarding cost effective health insurance program options. Our work plan also requires staff to spend time educating families on how to navigate the health care system, how to maximize the health insurance benefits, and how to resolve issues preventing enrollment and utilization. Health Promotion
Through a variety of grass-roots community programming activities, our Health Educators and Lay Health Workers (Promotores) make lasting relationships deep withinSanta Clara County's diverse communities. They provide care management, important health education, referral services, and a bridge to the local health care system. Focusing on underserved communities, we work to improve access to, and awareness of, health and wellness education and services.

Promotores de Salud or Lay Health Workers are public health professionals, community leaders, or volunteers who provide a variety of services related to health promotion and education.  These volunteers come from and are a part of the communities in which they serve. As a result, Promotores understand what is important to the community, they communicate in the language of the people they serve, and incorporate culturally competent practices to best serve the community. It's a community action model that empowers individuals to improve the health of their friends, family and neighborhood.
