Are you struggling to lead a healthful life with a chronic disease or condition? Do you need resources to help care for an ailing friend or loved one?

The Health Trust’s Chronic Disease Self Management Program is an evidence based program that provides people living with chronic condition(s) the tools to take charge of their condition and live healthier and more independent lives. Class curriculum covers chronic conditions including heart disease, asthma, arthritis, diabetes, COPD and HIV/AIDS.

Locations

These workshops are offered throughout San Jose and Santa Clara County at collaborating community based organizations including senior centers, low income housing and public libraries.
