![]() Are you struggling to lead a healthful life with a chronic disease or condition? Do you need resources to help care for an ailing friend or loved one? The Health Trust’s Chronic Disease Self Management Program is an evidence based program that provides people living with chronic condition(s) the tools to take charge of their condition and live healthier and more independent lives. Class curriculum covers chronic conditions including heart disease, asthma, arthritis, diabetes, COPD and HIV/AIDS. Locations These workshops are offered throughout San Jose and Santa Clara County at collaborating community based organizations including senior centers, low income housing and public libraries.

Diabetes Self Management Program(DSMP) A self-management program for people living with diabetes. The workshop also benefits individuals who care for someone living with diabetes. 1) techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, and emotional problems such as depression, anger, fear and frustration; 2) appropriate exercise for maintaining and improving strength and endurance; 3) healthy eating 4) appropriate use of medication; and 5) working more effectively with health care providers. Tomando Control de su Salud A self- management program in Spanish for people living with one or more chronic conditions. The Program is not a translation of the Chronic Disease Self-Management Program, but developed separately in Spanish. Better Choices Better Health Online Workshops The Health Trust will soon also offer Better Choices Better Health workshops online. Sessions last six weeks and can be joined from any computer with an internet connection. Participants log in on their own schedule a couple times a week to interact with their workshop group of approximately 25 people. Leaders are trained staff, many with chronic health conditions themselves, and participants can remain anonymous. Class curriculum can include, but is not limited to, diabetes, arthritis, high blood pressure, heart disease, chronic pain, and anxiety.
