Since 1987, when 394 Santa Clara County residents were diagnosed with AIDS, the picture of HIV/AIDS in our community has changed. Today, there are nearly 3,000 individuals in this county who struggle against its debilitating and life threatening effects. Although the number of new AIDS cases has declined steadily due in large part to case management, the disease continues to be a giant, menacing problem.Sadly, HIV infection rates continue to climb.

Meanwhile, cases have become far more complex as patients live longer and experience multiple, complicated needs. Without the case management services provided by The Health Trust’s AIDS Services, HIV/AIDS victims are at serious risk of losing jobs and health benefits, becoming malnourished and depressed, stopping their medications, turning to drugs, and even losing their ability to recognize the importance of practicing safe sex.

Serving Those Who Need Us Most
Of the more than 800 HIV/AIDS sufferers who turn to The Health Trust AIDS Services for help, two-thirds have multiple diagnoses, such as substance abuse or psychiatric problems. Almost 90 percent live in poverty, and many are homeless. The Health Trust AIDS Services case management model was formed to serve the neediest of the needy—who are nearly all of the complex HIV/AIDS cases in Santa Clara County.

A victim of HIV, Miguel served time in jail for drug abuse. When he was released he was angry about his diagnosis and deteriorating health. Miguel didn’t care what happened to himself or anyone around him. Fortunately, before leaving jail, he saw a social worker from THT AIDS Services who arranged for him to meet regularly with another social worker and nurse. Together, they helped Miguel keep his doctor’s appointments, stay on his medications, enroll for Medi-Cal benefits and find a job and permanent housing. They also encouraged him to enter a drug treatment program and pay attention to practicing safe sex.

By focusing on empowering patients and families to solve their day-to-day needs, the professionals at THT AIDS Services enables them to see the bigger picture, including the need for prevention. This change in attitude has a fundamental effect on fighting the spread of HIV.

Community Support is Critical
Today, the number of people who benefit from the unique services offered by THT AIDS Services continues to grow. In order to continue providing ongoing case management and stop the spread of AIDS, it is vital that the program receive financial assistance from the community. Your support is especially important, government grants only pay 80 percent of the costs, thus requiring a 20 percent matching gift from the community
