Overview
Our Community Health Promotions team provides at-risk, low income families with health information and services to address their wellness and preventive needs. Bilingual and bicultural staff introduces health messaging to families in a community setting such as their homes or at locations where it is most convenient to them; such as, community centers, churches, and local community based organizations. Oral Health, Colorectal Screening Awareness, Diabetes and Chronic Disease Self Management are some of the topics that delivered free to the community. Some workshops are for several hours and cover the basics of preventive care, while others are quite extensive and the coursework is scheduled over a six-week period. The significance of our work is defined by the individuals who facilitate and lead these workshops. Staff recruits, develops and trains community members to participate and complete the necessary work to eventually deliver the curriculum specific to the aforementioned topics. CHP staffs provides technical support, skills mentorship, evaluation and quality assurance to ensure that the Participants and workshop Leaders improve the health and wellbeing of our community.

Frequently Asked Questions for Families

Q: In what languages can you present/deliver the health message or training?

A: We have staff that is bilingual and bicultural in English, Spanish and Vietnamese and can present information regarding any of the topics fluently. Depending upon the topic, however, the curriculum may only be available in English or Spanish.

Q: Who are eligible for your workshops?

A: With the exception of the Diabetes Self Management curriculum, none of our health education modules require participants to have a specific pre-existing condition.

Q: Where do you recruit your Participants?

A: Because our target population is at-risk individuals who are prevented from receiving adequate health care and information because of barriers to health such as income and their ethnicity, our outreach extends deep into culturally specific neighborhoods where families are impoverished. Our recruits are often from referrals from the many partner agencies serving similar populations, as well as word-of-mouth referrals from those who have successfully completed the workshops and have seen improvements in themselves, or the opportunities to improve their loved ones. 

Q: Who pays for these workshops?

A: These programs are free to the community. Since we receive fee-for-service reimbursements from health plans covering low-income families, our recruit strategies are aimed towards these community members. Also, based upon specific grants requiring us to serve targeted ethnic populations or service areas, we will find servicing locations such as Indian Health Center, Native TANF, and MACSA.

Q: What the specific duration for each workshop?

A: Our Oral Health and Colorectal Screening Awareness workshops are typically 1-2 hours, depending upon the group size. The Diabetes and Chronic Disease Self Management programs are six-week sessions, with each session lasting approximately 2.5 hours.

Q: What do the Participants receive from these workshops?

A: Outside of formal/informal education of the topic area, Participants often leave with a Certificate of Completion, reference handouts, incentive items (Medication CD, tooth brush, tooth paste, etc.), provider directories, and/or long-lasting friendships with other participants.

Q: What age do you have to be?

A: Only the Oral Health workshop is available to those under 19, all other workshops, due to the materials covered, required levels of comprehension, and ability to impact life changes, it is limited to adults and seniors.
