HEALTHcheck is one of several new services The Health Trust will provide to the community, all of which emphasize prevention and creating a healthy and safe environment to live, work, learn and play. It’s a online tool that provides customized, personal profile and recommendations on ways to reduce health risks and achieve wellness

What you can learn from HEALTHcheck

• A profile of your overall health and any risk factors from lifestyle, family history, and existing health conditions
• Estimates of your risk of developing health conditions
• Customized, personal recommendations on ways you can improve your health, prevent many conditions, and enjoy wellness
• Extensive health and wellness information – a knowledgeable source to answer your questions and provide reports on new studies and trends

How HEALTHcheck works

• Register as a HEALTHcheck participant.
• Complete the General Health Profile questionnaire. It will take about 10-15 minutes. If you don’t have time to finish, your answers will be saved and you can return at any time, login, and continue.
• When you have completed the questionnaire, a customized personal profile will be provided to you that assesses your health, identifies potential health risks, and outlines preventive care recommendations.
• You can revisit the site and take the questionnaire again to track your progress. HEALTHcheck will automatically save your answers and results for you, and fill in any answers that are duplicated in any of the other 50+ assessments and wellness tools on the site.
