ters of Inquiry (LOI) Instructions and Format Letters of Inquiry should be addressed to: Format/Content Applicant Background - A one-paragraph description of your agency or organization, including: 1) year founded, 2) mission, 3) annual budget and major funding sources, 4) total number of programs and staff, 5) number of people served annually, and 6) most recent accomplishments. In addition, tell us how your mission is aligned with the interests of The Health Trust. Project Synopsis - A one-paragraph description of your proposed project, including: 1) name and location, 2) primary goals, objectives, and timeframe, 3) key collaborators and funders, 4) the amount requested and how the grant will be used, and 5) how the benefits or impact of the project will be sustained beyond the grant period. Project Approach and Outcomes - A one-paragraph explanation of the 1) how your project directly aligns with one of THT's Wellness Initiatives and the specific goals and strategies it will address within that Initiative 2) the primary target group to be served by the project, 3) key project outcomes and how they will be measured, 4) evidence-based primary or prevention approaches that will be used to achieve those outcomes, and 4) how the strategies or services provided by the proposed project are unique or differ from those offered by other providers of similar services in our community. Key Contact - Provide the name/title, telephone/fax number and email address of the primary contact for the project. (This person may be contacted to clarify information in the LOI or to schedule a site visit with the agency.)
