Application Process for Health Partnership Grants

 

Application Instructions and Format

All grant requests to The Health Trust are initiated by a Letter of Inquiry (LOI) (Download Form).  Applicants must submit a LOI that provides a clear, concise description of the proposed project and how it directly aligns with one of THT’s three Initiatives.  LOIs must be submitted on the organization's letterhead in a minimum 12-point font size with 1" margins and cannot exceed three (3) pages.  LOIs will be accepted via regular mail, fax, or email (with electronic signature). LOIs that are not submitted in this format will not be considered.
